A completed medical form is required yearly for each child attending KUC.
Print the medical form and provide completed form prior to start of school. If a child’s birthday is after school starts, their medical form can be submitted after their annual physical in the fall.
The completed form can be dropped off at the Church office during office hours or mailed to the Church at the following mailing address:
